McCoy was born in 2227. His mother died when he was very young, and his father... well, Leonard doesn't discuss his father's death.

Following his divorce from his wife in 2255, who got "the whole planet" in the settlement, McCoy chose to enlist in Starfleet, despite his aviophobia. After all, he felt like he had nowhere else to go. He and James T. Kirk met onboard the shuttle for Starfleet Academy. It was friendship from the words 'I may throw up on you.' Leonard was already a fully qualified medic, and a few years older than most other recruits, leading him to feel like a damn old man.

McCoy spent three years at Starfleet Academy, befriending Kirk in the process. During the hearing on Kirk's actions, a distress call was received from Vulcan. All cadets, including McCoy, but with the exception of Kirk, who was under academic suspension, were immediately activated for duty. McCoy was assigned to the USS Enterprise as a medical officer. He assisted Jim in his desire to go on board the Enterprise also, by infecting him with a small virus and taking him onboard, using his right as his attending physician to do so. He replaced Doctor Puri, the Enterprise chief medical officer, when Puri was killed in the battle with the Romulan Nero.
